Likelihood is where safeguards live and where most risk assessments go wrong.


Should you reduce likelihood because you have a relief valve? What about an operator response to an alarm? How do you account for a safeguard that works 99 times out of 100 but fails on the 100th?


This session covers how to assess frequency with safeguards in place, why "unlikely" is not a number, and how to stop your team defaulting to gut feel when the data exists. Including the relationship between likelihood and LOPA.
